This will need to be submitted to ori@wku.edu.
Please use this form to make any changes to your protocol. This includes the addition of students or Co-PIs, transfer of project, protocol changes, or animal care alterations.
This needs to be submitted every 3 years of the projects lifecycle in order for continuation of the study. Research may not continue if your project is expired.
If your project has come to an end, meaning all data collection and analysis is complete you will need to closeout your project. Please email ori@wku.edu to notify when your project is complete.
If you are transferring ownership of your project to another PI or the animals in your care are being cared for by another person in your absence, please email ori@wku.edu to inform of this change.
