Update of Graduate Program form

Admitted/enrolled student can request certain modifications to their active student records:

• Add certificate or endorsement (or add a master's to an existing Psy.D. program)

• Remove certificate, endorsement, or degree program

• Swap program concentrations

 

Please note:

• Update form applies only to active graduate records

• A new application is required to add or switch a degree program to a new discipline

• A new application is required to add a degree program based on existing stackable certificates

• A new application is required to apply for a new graduate program (certificate, endorsement, degree)
