Counseling Request Form

Gatton Academy Counseling Request Form

This form is for any current Gatton Academy student who would like to request counseling services from a professional on The Gatton Academy's staff.  Once completed, a member of the counseling team will be in touch with you to schedule an appointment.

 

Please write several ranges of dates and times in order to facilitate a meeting. Times must be on Monday-Friday, from 9:00am to 5:00pm. You will be assigned a counselor based on mutual availability. Appointments are 30 minutes. For seniors who worked with a Gatton counselor last year, please indicate so in the box above.
