Formstack is WKU’s self-service solution for creating and managing advanced online forms. It allows departments and offices to build their own forms without requiring custom development, while still supporting more advanced functionality such as multi-step forms, conditional logic, automated notifications, file uploads, and data collection workflows. Formstack is commonly used when a form needs more flexibility, routing, or data handling than the CMS Forms Module.

Access & Pricing

The Formstack Suite platform has been purchased centrally by WKU ITS, with initial focus on the Forms module rollout.

Departments must pay for individual Formstack user license costs as follows:

• $240 annually per user for form editing permissions.
• $120 annually per user for login access to view form submissions only.

Embedding Forms on WKU Websites

Formstack forms can be embedded into WKU CMS pages using a dedicated component. This allows forms to be displayed within existing site layouts while still using Formstack for data collection and processing.
