Legacy Form Assets are Being Phased Out

The new Forms Module is now available, and all new forms must be created using this updated system.

Existing form assets will continue to function for now, but should be recreated or moved to the new Forms module to avoid disruption.

July 30, 2026 (Subject to change) – Existing Form Assets will no longer be editable
November 2026 – Form Assets will be fully removed from the CMS

Formstack Form Embed Component

Formstack forms can be embedded into WKU CMS pages using the dedicated component. This allows forms to be displayed within existing site layouts while still using Formstack for data collection and processing. Formstack is commonly used when a form needs more flexibility, routing, or data handling than the CMS Forms Module.

When embedding a form, always use the WKU Embeddable theme in Formstack to prevent conflicting styles and a duplicate header/footer. See the article Applying a WKU Theme to a Formstack Form for step-by-step instructions.

Form URL

To locate your form URL, go to your form in formstack and go to the settings tab. Under Form URL, copy the editable portion
