Testing from DELO Testing Center and for On Demand Courses

You should have provided your professor with an FNL in advance of scheduling this exam. You will also need to provide the respective office with a copy of your Faculty notification Letter that has been signed by the professor for that course.

For the DELO Testing Center, please send your form to delo.offsite@wku.edu or drop it off at South Campus, Academic Wing, C239. You can find more information on accommodated testing with the DELO Testing Centers at https://www.wku.edu/testing/accommodations.php. 

For an On Demand course, please send your form to ondemand@wku.edu or drop it off at South Campus, Academic Wing, C113.

1. Follow this link: https://www.wku.edu/testing/offsite-exam-reg.php.
2. Provide the Exam Information.
   • You will need the course ID #, exam #/name, request date and time for exam, and the instructor's name.
   • For the date and time, be sure to request the same date and time with both offices.
3. Provide your personal Contact Information.
   • This includes your name, email, WKU ID #, and phone number.
4. For the ‘Test Location’ portion please fill in the blanks as follows:
   • Proctor's First Name: Hayley
   • Proctor's Last Name: Rigsby
   • Administrative Title: Office Assistant
   • Institution Name: SARC

6. Be sure you have completed a request with BOTH the SARC Office and the DELO Testing Centers.
