Legacy Form Assets are Being Phased Out

The new Forms Module is now available, and all new forms must be created using this updated system.

Existing form assets will continue to function for now, but should be recreated or moved to the new Forms module to avoid disruption.

July 2026 – Existing Form Assets will no longer be editable
November 2026 – Form Assets will be fully removed from the CMS

Form Example

This is an example of a form with one of each form element type as well as validation options. Forms created in the CMS will use default styling unless the Custom Styles URL is applied. If your form does not match the appearance of this example, follow the instructions, Apply WKU Styles to a Form.
