Spirit Master Request Form

If you would like to request assistance of the Spirit Masters at your event, please fill out the request form below.

Duties and Responsibilities of Spirit Master at Event

Hold down the CTRL key (Command ⌘ on Mac) to select multiple options.

Spirit Masters' formal attire consists of a business suit and casual attire consists of black pants (women) or grey pants (men) and a WKU polo. Please choose type of attire for your event:

The contact should be the person who the Spirit Masters will communicate with before, during, and following the event:

* Contact Name

The contact's phone number must be a reachable number for the Spirit Masters to use before, during, and following the event (format as XXX-XXX-XXXX):

* Contact Phone Number

* Contact Email Address
