|Ed.D. Program Planning Worksheet||Form||Completed during EDLD 702 - Orientation Course to guide student program progression. This is NOT the official GS Program of Study (see below).|
|GS Program of Study Change Form||Form||Completed and submitted to the Graduate School at any time the filed and approved 'Program of Study' changes. If more substantive changes are required, please see the Graduate School website for the appropriate form.|
|Ed.D. Internship||Form||Should be initiated at the time internship is agreed upon. The form is submitted to our office only after it is completed in its entirety.|
|Conflict of Interest||Form||Committee Conflict of Interest (description: As you form your committee or if you already have a committee in place, please have each faculty member complete and sign this fillable form electronically regarding any conflict of interest with your dissertation study. Once you have collected the signed form from each committee member, please submit all of them together directly to the Ed.D. Program Manager)|
|GS Graduate Committee Selection||Form||After you submit Conflicts of Interest, please have all members of your committee sign this form and email the Ed.D. Program Director. Our office will file on your behalf with the Graduate School.|
|Ed.D. - Dissertation Proposal||Form||Students must complete an oral defense of their dissertation proposal (Chapters 1-3) before beginning data collection, Please consult with your dissertation chair to schedule this meeting and complete and submit this form to our office.|
|Final Dissertation Signature Page and Packet||
