   Application for admission to the Associate of Science in Nursing Program (LPN-ASN)

  Along with submission of the following application, you are required to submit the following items or your application will be incomplete:

 

  • HESI Nursing Admission Exam Scores (POLICY UPDATE:  HESI must be within 1 year from application deadline date)
  • Transcript (if you attended a college other than WKU)
  • LPN school transcript & LPN License

You can submit your documents in the following ways:

IMPORTANT INFORMATION REGARDING THE LPN-ASN APPLICATION FOR ADMISSION SUBMISSION:

Students admitted to the program are required to be enrolled in the correct "Admit Term". Any student enrolled for the wrong term will need to contact the Office of Admissions via email to have the "Admit Term" corrected before they can be registered for any classes. Please see the LPN to ASN Program main page for more details.

It is the responsibility of the applicant, not the ASN Program, to see that all required information is submitted to this office. Your application cannot be considered unless all records are received in the program office on or before the deadline.   Please note the email address you provide on this application will be the email address used to reach you.

Applicants must notify the program office with any changes in contact information. If the ASN Program office is unable to reach an applicant by phone or e-mail, the applicant will forfeit their position in the program. Acceptance letters and acceptance forms will be sent to the email address provided on the nursing application. Acceptance form must be returned to the nursing office by the required deadline date or your position will be given to an alternate applicant.

The student with transfer courses must submit an official transcript to the Office of Admissions, Potter Hall, Room 117 (270) 745-2551. Be aware that it takes a minimum of three weeks to process the paperwork.   Therefore, you must have your transcripts submitted at least three weeks prior to the application deadline or your application will be considered incomplete.

This application will be considered incomplete without the full/legal signature of the applicant.

English as a Second Language

As nursing involves public safety, requirements are in place to ensure that nursing students can understand, read and speak English well enough to safely exchange written and verbal information with other health care providers and with patients from a wide range of backgrounds. 

If you speak English as a second language please consult the nursing website for TOEFL requirements and consult with the program coordinator to be sure that you have the appropriate documentation for entry into the WKU Nursing Program.  More information regarding the TOEFL requirements.

Have you ever applied to the LPN to ASN program at WKU?

Are you currently a WKU student?

Have you previously attended WKU, but are not currently enrolled?

Have you attended other colleges?

Have you been enrolled in a school of nursing?

Students must complete both prerequisite courses: Anatomy & Physiology (BIOL 131 or equivalent course(s)) and Developmental Psychology (PSY 220 or equivalent course) prior to starting the program.

If both prerequisite courses are complete, please email a copy of your college transcripts to asnnursing@wku.edu.

Students must take the HESI Admission Assessment (HESI A2) prior to the application deadline.

The HESI A2 entrance exam is required to apply. Also, HESI scores must be dated within 1 year of the application deadline date.

Either answer is acceptable, however, emailing transcripts to asnnursing@wku.edu is preferrable.
