Environmental Health & Safety Reporting
Please report any environmental health, safety or injury concern on the appropriate form. If there is an immediate danger to life or health, contact WKU Police at 911 or 270-745-2548.
Title IX Reporting
Please use this form to report incidents of sex and gender discrimination, including sexual harassment, sexual assault, rape, dating and domestic violence, and stalking.
Student Conduct Reporting
Report any alleged violation of the code of conduct or students of concern.
Discrimination, Harassment, and Retaliation Reporting
Please use this form to report incidents of protected class discrimination, harassment, and retaliation. A protected class includes, but is not limited to, race, color, ethnic origin, national origin, religion, age, disability, etc.
Information Technology Reporting
You can report issues with information technology, such as a problem with your computer or a technological security threat.
Ethics & Compliance Reporting
Anonymously report ethics concerns, fiscal misconduct, or compliance violations to the WKU Office of Internal Audit.
Academic Dishonesty
Report academic misconduct, dishonesty, cheating, plagiarism, and any other behavior that violates the basic principals of integrity and honesty.
