2013 Summer Camp Registration Form

Please complete the form below. If registering for multiple children please use a separate form for each child.

Clay camp is full, if you wish to be placed on a wait list call 270-745-2594.

Chinese Culture Camp registration is closed.

PAYMENT

Member fee per camp is $108.   Non member fee per camp is $120.

Full payment must be made at time of registration.

When paying with check please make the check out to the Kentucky Museum. Mail check along with parental consent form and camp rules to:
