Qualifications for Nomination

 

1. Must have been born in the Commonwealth of Kentucky or a current resident of the state.
2. Has made a difference in the lives of the citizens of the nominee's city, county, state, region, nation or world.
3. Is willing to attend and speak at an award ceremony in June or July.
4. Must be nominated through the proper nomination process listed below.

Please also provide two additional letters of nomination, why they deserve this honor, professional and educational background, and any supporting documents.
