The Post Health Behavior Questionnaire is available from Wednesday, April 18th until Tuesday, May 8th.
To complete the survey, sign onto your WellU® page and the survey will automatically pop up on your screen.
Upon completion of the Post Health Behavior Questionnaire you will earn:
- 2 Fitness Module Credits
- Your name will be entered into a raffle for the chance to win a one of three $100 Visa Gift Cards. Drawing for the gift cards will be Wednesday, May 9th.
