Production Report Form

Thank you for filling out this Production Report Form. We ask that one supervisory person (production stage manager, stage manager, director, producer, or faculty supervisor) complete this form for every show produced or co-produced by WKU Department of Theatre & Dance. This includes all public performances in the Russell H. Miller Theatre and Lab Theatre, as well as co-productions in Van Meter, children's theatre tour shows, Zoom performances, class showcases, recitals, staged readings, etc. If you're not sure, go ahead and fill out the form. Your answers help us keep track of important information that we use for recruiting, planning, budgeting, and reporting to the administration.
