Surplus Form: 

This form is now required for all Surplus Pick-Up Requests or Scheduled Drop-Offs.

Before completing this form, please submit a Work Order for a Surplus Pick-Up through Maintenance Connection: WKU's campus-wide maintenance request system. Registered users may do so here.

The Surplus Form below provides Surplus and Recycling staff with the necessary information to properly manage, track, and report Surplus assets, maximize asset value to the university, and boost efficiency. Thank you for helping to improve Surplus procedures!

Building, Room#: Please be as specific as possible. If someone moves the items after submitting this form, please email elizabeth.gafford@wku.edu, so we will be able to find them in their new location.

Please list these in order of the items listed above. Inventory Control Numbers are very important to report to ensure accuracy of insurance and financial records. Please look carefully for these numbers on all items and record them accurately. If no ICN is present for an item, please write, "Not present."

Example: Is this item gently used, new, damaged, broken, etc.?

For Example: Renovation project, item is no longer needed, damaged/broken, etc.
