Funny Monkey Example Form


Left Navigation Info

Navigation menus are done in <ul> and <li> tags.  The HTML code is set up as follows:

<li>Menu Item 1</li>
<li>Menu Item 2</li>

Left Column Info

Not only can you have your nav in the left (column 1) but you can also add content and images. Please note that anything you put in the left column will show up on ALL of the pages within your site.

This is an example form created through OUCampus's LDP Form Builder.  Please feel free to visit the self help page for more information on how to create the forms. PLEASE DO NOT FILL OUT THIS FORM AND EXPECT A RESPONSE.  THIS IS JUST AN EXAMPLE.

Everything below this line can be created through the Form Builder.  Some of elements below are special elements that we have bulit into the code.  You will need to visit the LDP Special Elements page for more information on how to do those.

Must be a valid email address.

First and Last. (Example of helper text)

You can display date, time, or both and select a default.

Some information text. You can add text in between form elements. (Added through Asset)

This is an example of a Multi-Line Text Field.

Radios and Checkboxes
These elements are grouped together by a dotted outline box called a fieldset. Learn more about Advanced Form Options.

You can only select one radio at a time.

You can select multiple checkboxes.

This Drop-Down Select is auto-populated by an advanced option so you don't have to enter each state.

Hold down the CTRL key (Command ⌘ on Mac) to select multiple options.
