WKU Archives Monthly Statistics Form

Note: If you are filling out this form for someone else please type their name beside your name in parenthesis in the Name field, but type only your email in the E-mail field.

If you are needing to submit fiscal year WKU Archives statistics, please go to the WKU Archives Fiscal Year Statistics form to submit those. This form is for monthly statistics submissions only.

e.g. If Jane Doe submits a form for John Doe the Name field would say Jane Doe (John Doe), but the E-mail field would simply say Jane.Doe@wku.edu.

After clicking submit the form should disappear and a green confirmation message box should appear in its place if the form mission was accepted. If this does not occur please scroll back to the top of the form to check for errors and then try to submit it again. Error messages will appear in a red box at the top of the form.
