Conference Registration & Cancellation Requests:
Must be made in writing to _________________ by October 21, 2021. A refund of all
fees, less a $20 administrative fee, will be given for cancellations received by this
date. Refunds will be returned to the original payer and in the original method of
payment. Please note that refunds will not be given for no-shows. All requests for
exceptions to the cancellation/refund policy must be submitted in writing by the registrant
with appropriate documentation no later than October 21, 2021. After this date, no
refund considerations will be made. A transfer of your full registration to another
individual is permitted prior to the conference start date by submitting a written
request to _________________.
COVID-19 Information:
We continue to monitor the impact of COVID on federal, state and local rules and regulations
and will make adjustments accordingly. We want to continue to deliver valuable professional
development, while keeping your health and safety a priority. We plan to adhere to
all CDC guidelines and any laws, regulations, or executive orders in place at the
time of the conference. Under CDC guidelines (as of June 27, 2021), the CDC states
that fully vaccinated people do not need to wear a mask or socially distance in most
indoor and outdoor settings in the United States — unless required by federal, state,
local or tribal laws. People are considered fully vaccinated at least two weeks after
receiving the single dose of the Johnson & Johnson vaccine, or the second dose of
the Pfizer or Moderna vaccines. Those who are not fully vaccinated, however, should
continue to wear face masks indoors and in some outdoor settings. We request all conference
participants to voluntarily adhere to any and all requirements.
If the In-person Conference is Cancelled Due to Public Health Concerns:
In the unlikely (but possible) event that the in-person Symposium is cancelled due
to public health concerns, such as a spike in the number of COVID-19 cases due to
a new variant or otherwise, the Symposium will transition to an online conference.
In such event, 50% of the registration fees paid will be refunded to all attendees,
as the Symposium continues online. Full refunds will be provided to any attendees
who cancel, in accordance with the policies state above.
