Important Forms

Application for COMM 595: Independent Study in Communication

There are four major forms all graduate students must complete: Form C, Form D, Course Change Forms, and the Application for Graduation. All forms should remain paperless, so you'll need to create a Digital Signature to sign them before sending them to your advisor. Here is a link to instructions on creating a digital signature on your computer.

Form C (for new students):
This form is your degree program and should be completed by all new students in their first semester. If you are in your first semester or just finishing your first semester, please make an appointment with your advisor to complete the Form C. Here is the link to Form C.

Course Change Forms (for graduating students):
If you have already completed your Form C but have taken different courses from what was included on the form, you will need to complete a Course Change Form. Please complete the form electronically and forward to your advisor for approval. Here is the link to the Course Change Form.

Form D (for students who have earned more than 15 hrs):
Once you have completed 15 hours of coursework and at least one of the research tool courses (as recorded on your Form C) with a "B" or higher, and maintained a 3.0 GPA, you need to complete Form D: The Admission to Candidacy. This is a traditional form that serves as an agreement between you and the university that you are going to successfully complete the program. This form triggers Graduate Studies to audit your degree program (Form C) to verify that you are on track to graduate. Please complete this form electronically, provide your digital signature, and forward to your advisor for approval. Here is the link to Form D.

Application for Graduation (for those graduating in December, May, or August):
Once Form D is submitted and approved by the Graduate Studies office, you need to complete an application for Graduation which is accessible via TopNet. The fee for graduation is $55.00. Your application should be completed the semester before you intend to graduate. Here are the steps:

1. Sign on to Topnet with your WKU ID and PIN.
2. Follow the 'Student Services' link.
3. Follow the 'Student Records' link.
4. Follow the 'Application for Graduation' link and fill out the form provided.
