WKU Sport Clubs requires that all sport clubs have complete and accurate medical information filed in the event of injury. These forms must be typed on a computer or neatly handwritten. If you have any questions about this process, please contact one of our club officers as soon as possible. We can not allow any individual to take part in practice until we have this information for that person.
