Faculty member — Please fill out and submit this form to help us accommodate your testing needs.

• If this is a makeup exam request— DO NOT USE THIS FORM. Instead, please use the Makeup Exam Request Form.

• Also very IMPORTANT: Please do not put your exam password on this form. Instead send an email to dltesting@wku.edu with the Course ID and test number in the subject line. Thanks!

For an online exam in Blackboard, has Auto Submit been selected for the exam?
(NOTE: If you select yes for auto-submit, it will save the exam for the student and submit it when the student reaches the max. allotted time whether the student has completed the exam or not. If you do not want your students stopped at the max. allotted time, please do not select this option. Please also be aware that this option may cause issues within the testing center if there is an emergency such as a fire alarm because your student will lose time during the exam and have to be reset and start the exam again.)

For an online exam in Blackboard, has Force Completion been selected?
(NOTE: The DL Testing Center staff recommends that you do not set force completion on your Blackboard exams. If force completion is not selected it allows us to immediately re-enter a student into an exam (with the password) in the event of a technical problem. The student can then resume the exam instead of restarting again. )

For Paper Exams, do you want the tester to be stopped at the max. allotted time?

Important Info for Faculty Requesting Exams
  • All students taking this exam must schedule their exam with the DL Testing Center. Students taking exams at other proctored locations must inform the DL Testing Center AND make a testing appointment with their chosen remote location.
  • Online Exams: Password for this exam must be given to the DL Testing Center no later than 4 days prior to the first day the students may take the exam.
  • Paper/Pencil Exams: Paper exams for all students must be given to the DL Testing Center no later than 10 business days prior to the first day the students may take the exam.
  • If giving a paper exam, we will deliver your exam to you once they have all arrived or you may request to pick them up instead.
  • All unused exams will be returned to the instructor unless other instructions are given.
