WKU Study Abroad Forms

• Study Abroad Initial Interest Form
• Use this form to let WKU Study Abroad and Global Learning know that you are interested in studying abroad. Once you have completed the form, e-mail it to study.abroad@wku.edu. In the e-mail, be sure to include dates and times you are available for an advising session.
• Study Abroad Program Selection Form
  
• Use this form to start the application process to study abroad with WKU. Please note that this form also serves as the World Topper Scholarship Application. Once you have completed the form, you may e-mail it to study.abroad@wku.edu.
• Faculty Reference Form
• Two faculty reference forms are required for the World Topper Application, they are also required if you are applying for a WKU Exchange Program. This form may be submitted to the Study Abroad office directly by the faculty member.
• Study Abroad Course Amendment Form
• Use this form to amend any course change while abroad.
• Study Abroad Program Evaluation
• After the completion of your program, let us know about your experience by completing the evaluation survey. All responses will be kept confidential and personal information will not be shared with third parties.
