Upon receipt of your form, you will be registered for DanceDay on Sunday, December 6, 2009. If you have requested extra tickets, your seats will NOT be reserved until we have received payment. Please mail your payment to the address listed at your earliest convenience.

If you would like to see our production of Winter Dance, 2009, please indicate if you will need 1 or 2 complimentary tickets.

Additional tickets may be purchases @ $10.00 each.
