Privacy Statement
At WKU Health Services, we are committed to treating your protected health information in a responsible and ethical manner in accordance with HIPAA guidelines.
We will only release your information to parties that you have designated as authorized recipients on your Authorization to Release Restricted Health Information Form. No other parties, including parents, and/or guardians will be allowed access to your records without your prior consent, unless state or federal laws mandate the release of the information.
We will only contact you via the methods, phone numbers, and addresses that you authorize on your Acknowledgement of Receipt of HIPAA Privacy Practices form.
