Personal Training Appointment Procedures:
The Health & Fitness Lab will provide you an autobiography and contact information of the personal trainer. You can meet the personal trainer for a free consultation session before purchasing sessions.
The fee must be paid at least 24 hours before the training session in the Health & Fitness Lab. The Health & Fitness Lab accepts the following payment types: Cash, Check, and VISA & Mastercard.
Payment is non-refundable.
You will be required to will out a medical history form and personal training questionnaire. You may be instructed to provide us a medical clearance note from your family physician. If so, it may be faxed to us at (270) 745-6534.
Your appointment will be scheduled anytime during the Preston Center hours of operation. The time of the session is agreed upon between you and the trainer. If a session needs to be cancelled for any reason other than emergency, a 24-hour notice must be given to the trainer or you will be rendered responsible for the payment of that session. If you are more than 15 minutes late, the session/appointment will be forfeited.
Arrive at your appointment prepared to exercise. Your attire should be loose fitting (i.e. t-shirt, shorts) and you should be wearing close toed sport shoes. Please call our office if you have any questions.
