Hardin Planetarium Camp Registration Form
Spring, 2014 Science Investigation Camp
March 31-April 4, 2014, 8 a.m. to 12 p.m.
Easy 4-Step Registration Procedure:
- Please complete the Registration form below.
You may either fill in the form below and submit it online; or print the form, fill it out; and then fax it to 270-745-2014, mail it or drop it off in person. Receipt of the registration form holds a spot in the camp provided that payment in full is received before the late registration deadline of March 21, 2014.

- Please pay in full at the time of registration. Registration will not be considered complete and a spot for the camper will not be held until payment in full is made.
- After the late registration deadline of March 21, 2014, the late registration fee of $150 (instead of $120) will apply. Please be sure to read our Cancellation Refund Policy.
- Open and download the Parental Consent form, fill out, sign and submit. You may either attach the page to an email, fax it to 270-745-2014, mail it in, or drop it off in person (see below).
- Open and download the Camp Rules form, fill out, sign and submit.

Paying and Submitting forms online:
Credit card payment and registration form should be submitted on line.
Alternatively, the Registration form and all other forms may be printed and emailed as attachments to email@example.com.
When paying with a check please make the check payable to: "Western Kentucky University, Hardin Planetarium." Please make one payment for the FULL amount. Registration is not complete, and a place in the camp will not be held without payment in full. (Please note: Checks returned for insufficient funds, closed account or stop payment will incur a $20 charge. )

Dropping off in person:
Bring Registration form, check, Parental Consent form and Camp Rules form to the Hardin Planetarium, located at the intersection of State Street and College Heights Blvd. in Bowling Green KY. It is best to call ahead (270-745-4044) to ensure that the building is open as we are sometimes in the field, and don't always keep regular hours. In the event that the Planetarium is closed, registration materials can be taken next door to Thompson Complex Central Wing, the Physics and Astronomy Office, TCCW 246, 2nd floor, during normal business hours.
