Hardin Planetarium Camp Registration Form
Summer, 2013 Science Investigation Camp

July 8-12, 2013, 8 a.m. to 12 p.m.

Easy 4-Step Registration Procedure:

  1. Please complete the Registration form below.
    You may either fill in the form below and submit it online; or print the form, fill it out; and then fax it to 270-745-2014, mail it or drop it off in person. Receipt of the registration form holds a spot in the camp provided that payment in full is received before the late registration deadline of June 28th, 2013.
  2. Make a payment .
    We're sorry but we can not currently accept Credit Card Payments. Please send a check in the mail or drop it off in person (see below). After the late registration deadline of June 28th, the late registration fee of $150 (instead of $120) will apply. Please be sure to read our Cancellation Refund Policy.
  3. Open and download the Parental Consent form, fill out, sign and submit.
    You may either attach the page to an email, fax it to 270-745-2014, mail it in, or drop it off in person.
  4. Open and download the Camp Rules form, fill out, sign and submit.
    You may either attach the page to an email, fax it to 270-745-2014, mail it in or drop it off in person.

 

When paying with a check please make the check payable to: "Western Kentucky University, Hardin Planetarium."

 

Submitting forms online:

Registration form may be submitted on line.

Alternatively, the Registration form and all other forms may be printed and emailed as attachments to hardin.planetarium@wku.edu.

 Dropping off in person:

Bring Registration form, check, Parental Consent form and Camp Rules form to the Hardin Planetarium, located at the intersection of State Street and College Heights Blvd. in Bowling Green KY. It is best to call ahead (270-745-4044) to ensure that the building is open as we are sometimes in the field, and don't always keep regular hours. In the event that the Planetarium is closed, registration materials can be taken next door to Thompson Complex Central Wing, the Physics and Astronomy Office, TCCW 246, 2nd floor, during normal business hours.
