Shin Megami Tensei

Everytown USA, or at least thats what the characters thought they were from. A sinister looking plot in a hospital leads to a door to the abyss, and a man in a wheelchair gifts the players with strange circlets. Now they are in a world that is slowly going to hell, trying to survive and figure out what they can do in the process.
