Musician Form

WORD Version of Form

Dear Participant:

"Diversity Rocks" is a unified event sponsored by various organizations on the WKU campus that will feature campus wide and local talents.  Your participation in "Diversity Rocks 2004" is greatly appreciated.  Let's celebrate diversity!

Musician Guidelines:

  • Diversity Rocks Council will provide some sound equipment; however, please contact Molly Kerby via e-mail (molly.kerby@wku.edu) or by phone (270) 745-6103 with your band's needs prior to the event.
  • No lyrics denigrating to any segment of humanity.
  • Volume will be monitored to prevent disturbing classes.
  • Set up time should be minimal because of the tight scheduling of musical events -- please tune prior to the performance (space will be provided).

Non-Discrimination Agreement:

Each musician must sign the following agreement:

I/We attest that (group name)__________________________ does not discriminate based on gender, race/ethnicity, sexual orientation, religion, age, creed, language, or any other differences found in the Human Race.
