to Internal Audit
You may use the form at the bottom
of this page to report an issue to Internal Audit. We welcome
your input. We rely on your knowledge of activities within the University
to help us assess whether the controls established by University management
are effective in promoting the goals and objectives of the University.
If you wish to remain anonymous,
please leave the name, address, and telephone number fields blank.
Be sure to provide us with enough detailed information regarding the issue
so we may conduct an inquiry. We will only investigate legitimate University
issues concerning misuse of University assets, failure to comply with University
policies, etc.
Examples of issues to be reported on this form:

Suspected fraud

Suspected use of University property for non-University
activities or personal gain

Departments depositing money into their own checking
account rather than through the University's Cash Receipts Transmittal

Suspected expenditures of departmental funds for
personal use or other non-allowable expenses

Suspected failure to comply with University policies
and procedures, or state or federal laws and regulations
Examples of issues NOT to be reported on this form:

Sexual harrassment (please report this directly
to the Equal Opportunity Office (745-5121), Human Resources (745-5360),
the Dean of Student Life (745-2791), or the Office of the Provost (745-2297))

Crime (please report this directly to the
WKU
Police Criminal Investigations Division or call 911 for an emergency)

Personal grievances with other employees or superiors (please follow
guidelines established in the Personnel Policies and Procedures Manual as
set forth by Human Resources)
