Report an Issue to Environment, Health & Safety
This form is to be used to report potential environmental or safety hazards.
Examples of issues to be reported on this form:
- Potential fire hazards.
- Slip, trip and fall hazards.
- Improper removal and disposal of asbestos or any other hazardous waste.
- Improper storage of hazardous materials, such as, flammables, toxic and corrosive substances
- Failure to follow safe work practices.
Examples of issues NOT to be reported on this form:
- Sexual harassment (please report this directly to the Equal Opportunity Office (745-5121), Human Resources (745-2071), the Dean of Student Life (745-2791), or the Office of the Provost (745-2297)).
- Crime (please report this directly to the WKU Police Criminal Investigations Division or call 911 for an emergency)
- Personal grievances with other employees or superiors (please follow guidelines established in the Personnel Policies and Procedures Manual as set forth by Human Resources)
