You can receive a 3-month supply of your medication for the cost of a 2-month supply. Your physician will need to write the prescription for a 90-day supply. Once you have the written prescription, complete an Anthem Rx Direct Order Form which you can pick up in the Department of Human Resources. Then mail the original copy of the prescription and the order form in the envelope supplied with the order form. Delivery time is normally 7-10 working days after Anthem receives your order.
