IMPORTANT - Please read these instructions carefully before completing the online enrollment form:

• HEALTH - ALL employees must re-enroll in a Health Plan for 2010, including those who opt out of the WKU Health Plan and receive the waiver funds
  
  

  • Please list all dependents you will cover on the Health Plan for 2010

  
  
• FLEXIBLE SPENDING ACCOUNTS - If you participate in the voluntary medical or dependent care flexible spending accounts, you will need to indicate such on the form and enter your annual election amount for 2010.

• DENTAL - You DO NOT need to complete anything in the Dental section unless you are making changes for 2010

• LIFE & DISABILITY – You DO NOT need to complete anything in the Life Insurance or Disability sections unless you are making changes for 2010

• If you are increasing/decreasing your optional life insurance amount for 2010, please indicate the TOTAL AMOUNT of coverage you are electing for 2010 (NOT the amount you wish to increase/decrease)
