The Blue Vision network is a national network that consists of more than 13,000 providers including optometrists and ophthalmologists as well as more then 500 LensCrafters retail stores nationwide. Benefits include:

• Eye exam requires a $10 copayment for in network providers
• Supplies, lenses and frames have a $20 copayment ($120 limit on frames)

Non Network Benefits include:

• Non network visits are reimbursed up to $42
• Single vision lenses are reimbursed up to $40
• Bifocal lenses are reimbursed up to $60
• Progressive lenses are reimbursed up to $60
• Trifocal lenses are reimbursed up to $80
• Lenticular lenses are reimbursed up to $125
• Frames are reimbursed up to $45
