Anthem Blue Cross and Blue Shield is the provider of Western Kentucky University's self-insured group health plan. Anthem's administrative offices are located in Louisville, Kentucky. The toll free number is 1-888-650-4047. The customer service number and mailing address for claims are located on the back of the member's Anthem identification card. Eligibility - Comprehensive health care benefits are available to fulll time employees and their eligible dependents. Employees may elect to cover lawful spouses and unmarried dependent children from birth to age 24 as long as the dependent child is claimed as a federal tax exemption. Student status is not a part of the criteria for coverage. Coverage for a dependent turning age 24 is continued through the end of the year in which the 24th birthday occurs. Plan Design - There are three plan designs from which to choose when selecting your health insurance option:
All 3 plans utilize the same network. The differences in the plans relate to the amount of the deductible, copays, co-insurance, number of visits for certain services and out-of-pocket maximums. For specific information with regard to covered services under each plan, please refer to the 2006 Benefits Manual. Once a health plan is selected, it remains effective for the full plan year (calendar period). Changes in enrollment are only permitted during a plan year for qualifying events such as marriage, divorce, death, legal separation, birth or adoption of a child. Otherwise, changes are made during open enrollment and effective on January 1 of the next calendar year. Cross Reference (CR) If both spouses are employed at the University in benefits eligible positions, each spouse will receive the University contribution thereby resulting in a decrease in premium. Wellness at Western Kentucky University - WKU supports a variety of programs and opportunities intended to improve the overall health of employees. Employees are offered the opportunity to be members of the Preston Center for a minimal monthly charge based on their annual income.
