In effort to improve the services the BSA staff provide to each of you on
a daily basis, we are implementing this 'Customer Concern Form' for you to
use to let us know about issues or concerns in regards to our cleaning services provided in
your assigned building(s).
Please feel free to let us know of area(s) that you feel needs attention, or that could be
improved in some way. This information will be considered confidential, but will be used to improve
our services provided to the University campus community.
Names are not required, but would be helpful when responding to your concerns.
