Degree Program Checklist

You may find this checklist helpful in proofing advisees' degree programs and/or may give a copy to advisees before they complete their program. You may also find it helpful to design an addendum for your major/minor advisees.

Return this memo and your degree program materials to your advisor after you have made the requested changes and/or attached additional information. Thank you!

    • Your minor or second major department head (2nd major advisor also) must sign your program before it can be approved by this department.
    • The Registrar's computer record shows that you have not officially declared _________ as your major/second major/minor. Please sign the attached form and take it to the Advising Center, Rm 226 CH.
    • Please indicate courses by departmental abbreviation and course number, not by title.
    • Please finish filling out your program.

    • You do not meet residency requirements in your major/second major/minor (1/3 of hours in each must be taken at WKU).
    • One half of hours in major and minor must be upper division (300-499).
    • Transfer courses must meet the following requirements:

      ___The same course taken at two different schools can only count once toward graduation.
