Admit/Exit Slips Example 1

You are to write about the following things.
Your completed work should be about one page long.
  1. Tell me about at least three things you like in math class.  Be specific about why you like these things.
  2. Tell me about at least three things you dislike in math class.  Be specific about why you dislike these things.
  3. Tell me how a math teacher can help you learn.  Is there something specific I should know about teaching you math?
  4. Complete the following sentence.  Be sure to tell me how you felt about it.  One time in math class...
Sample A
  1. In math I like figuring word problems, writing on the boad to solve problems and division.  I would like to have a day where the kids have assignments over something then we teach it to the rest of the class.  We should play simple games to learn harder parts.
  2. I didn't like to sit and listen to the teacher talk all class.
  3. I don't no how you can help me learn.
  4. One time in math class I failed a test.  It made me feel bad.  I am no good at math.
Sample B
  1. I like adding, subtracting, and fractions because I am good at them.
  2. I hate word problems, lots of homework, and tests because I don't like them.
  3. You should not give any homework and we should have partners for tests.  Be sure to give me someone smart.
  4. One time in math class this girl sneezed and farted at the same time.  Everybody laughed because it was so funny, but she cried.  Now we call her achoobrrrrr.   She still cries.
Teacher Comments

This prompt contains more questions than I normally use for exit slips.  I use it as a get acquainted activity that lasts about 20-30 minutes on the first full day of class.  Student's turn in their responses as they leave class.  The length is set to encourage students to write fully, but not go on for pages and pages.  I do not take off for shorter assignments as long as all questions have been answered.  If any questions are unanswered, I return the paper to the student and ask them to try again.  This assignment usually provides insight into my students and sometimes what they chose to share is very poignant.  Homework and tests are perennial topics for dislike even though I discuss their purpose and necessity with the class prior to the assignment.  At the next class meeting I usually post and read a transparency of all the likes and dislikes.  I try to be positive about the likes and discuss the real life necessity of the dislikes.
