Experience Rambles without an apparent goal of moving or communicating with the  reader. Poem adequately conveys a significant experience. Poem conveys a profound experience in a provocatively moving way.
Voice Speaker(s) not consistent in tome and viewpoint.

Speaker (s) consistent in tone but not in point of view or vice versa.

 A range of emotions, some in crescendos, are unmistakably conveyed.

.Form No attempt at form, abstract or other. Form is present but not especially suitable for the message of the poem. Form is present and is conspicuously or subtly appropriate to the message of the poem.
Literary Devices No visual imagery or other literary devices used. Poem has some imagery and figures of speech. A rich and engaging mix of literary devices appropriate to the length of the poem are used.
Language Simple, not well thought out.

Careless placement of words.

Language of poem shows some thought and consideration.

Words are somewhat deliberately chosen and placed.

Transition Emerged product misses the point of the advertisement on which it is based-- has no semblance of conversion to poetic form. Poem is very speciously (look it up) based on the content of the ad though in acceptable poetic form. Thrust of advertisement is conveyed accurately in exemplary poetic form.
Publication Not polished enough to be accepted anywhere, even afer numerous submissions. Published locally i.e. school anthology, bulletin, Celebrations, etc. Printed in highly competitive journal, magazine or other such publication.
Note:  As usual, correct spelling, grammar, punctuation and usage characterize the poetry.  A "super" level of performance will be basically surface error free.  An "emergent" level will have numerous errors which interefere with the  communication of the central idea of the poem, far exceeding the level and complexity of the piece.
